Uncontrolled hypertension (SBP > 160)

Abnormal renal function

Abnormal liver function

Previous stroke

Bleeding history or predisposition

Labile INR (on warfarin)

Age > 65 years

Antiplatelet or NSAID use

Alcohol ≥ 8 units/week

HAS-BLED

Low to moderate bleeding risk — continue anticoagulation with routine review.

1 point each: Hypertension (uncontrolled), Abnormal renal or liver function, Stroke, Bleeding history, Labile INR, Elderly > 65, Drugs or alcohol
