Heart rate > 100/min (1.5)

Immobilisation ≥ 3 days or surgery within 4 weeks (1.5)

Previous DVT or PE (1.5)

Haemoptysis (1)

Active malignancy (1)

Wells (PE)

PE unlikely — a negative age-adjusted D-dimer safely excludes PE without imaging.

Do not use D-dimer alone when PE is likely.

Weighted criteria totalling 0-12.5; ≤ 4 PE unlikely, > 4 PE likely (two-level rule)
