Key recommendations
- AClass I
IV thrombolysis is recommended within 4.5 h of symptom onset in eligible patients.
- AClass I
Mechanical thrombectomy within 6 h for anterior circulation large vessel occlusion; 6-24 h with perfusion selection.
- BClass III/IIb
Do not lower blood pressure aggressively unless > 220/120 mmHg or thrombolysis planned.
